How Can Surgery Stop Migraines?

The treatment of migraine headaches through surgery was discovered by Cleveland, Ohio, plastic surgeon Dr. Bahman Guyuron. He found that many patients who underwent BOTOX® Cosmetic injections or brow lift surgery experienced significant relief from migraines. The headaches returned in the BOTOX patients after their treatments wore off, but not in the brow lift patients. Dr. Guyuron reasoned that both treatments disabled the muscles responsible for triggering migraine headaches; the improvements from BOTOX were temporary and those from surgery were long term.

Am I a Candidate?

When patients visit him for migraine surgery at his cosmetic surgery center in California, Dr. McNemar first determines whether the treatment is likely to be effective. He identifies the migraine trigger sites by systematically testing the following 4 known areas:

For most patients, 2 to 3 of these areas are trigger sites for their migraines.

All areas except the nasal passages can be tested using BOTOX injections; the nasal passages can be tested with nasal spray, as well as x-rays and physical examination. What to Expect

Dr. McNemar usually performs migraine surgery as an outpatient procedure while the patient is under sedation or general anesthesia. The actual surgery, which involves severing a portion of the muscle at each identified trigger site, depends on the location and number of trigger sites. When addressing forehead triggers, the surgery is similar to a cosmetic brow lift. For triggers in back of the neck, Dr. McNemar conceals the incision in the scalp. He targets nasal passages much like surgery for a deviated septum where the small incisions are concealed inside the nose.

Results & Recovery

After migraine surgery, patients commonly experience headaches for the first 2 weeks, which can be managed with pain medication. Depending on the location and number of trigger sites, most migraine surgery patients can return to their normal daily activities, including work, within 1 to 2 weeks.

Most patients notice improvements within 2 to 4 weeks after their migraine surgery. When forehead sites are treated the patient often attains the added benefits of a brow lift including improvement of forehead creases and eyebrow position.
